North Dakota Nurse License Plate

The North Dakota Center for Nursing in conjunction with the state’s nursing community, has developed the official license plate for the nursing profession! The plates are available for anyone wishing to show their support for nurses.

In order for the state to offer the nurse license plate, we need a minimum of 50 pre-ordered plates.

Whether you, a friend, or family member is a nurse, or you simply want to show your appreciation for nurses, purchase a plate today to and show your support for the most trusted profession in North Dakota and the United States!

How to Purchase

REMINDER: DO NOT SEND YOUR APPLICATION TO THE DMV. YOUR APPLICATION AND PAYMENT MUST BE SENT TO OUR MAILING ADDRESS. 

If you would like to purchase a nurse plate, please fill out the application form from the North Dakota DMV at: https://www.dot.nd.gov/forms/sfn54397.pdf and include your credit card information or send a $25 check written out to ND Department of Transportation, Motor Vehicle and mail these items to our mailing address below.

Your application and payment (check or credit card on the form) must be included to complete your application. Please send these items to us at:

If you would like to personalize your plate, that is an additional $25. You are able to check name plate availability at https://apps.nd.gov/dot/mv/mvrenewal/plate.htm

Application Deadline

Once we have collected 50 pre-ordered plates, we will send them to the North Dakota DMV. After the submission, the nurse plate will be available to anyone in North Dakota. We will be accepting applications until December 31, 2019 or until we reach 50 applications.
